How do I report a blocked storm drain?

How do I go about getting a culvert installed in the open ditch in my front yard?
Fill out the Culvert Request Form and submit it to the public works department. After your information is reviewed by the Engineering Department you will be contacted with information concerning what size culvert needs to purchased. Please do not purchase any culvert material until after we contact you. After the culvert material has been purchased and delivered at your home, contact the Public Works Department and a work order will be issued for our Street Maintenance crew to install the culvert at no charge to the resident.
